I just now read this piece. Thank you for it. I've delivered the presentation related to that article a few times now, and frankly, I can't help but get emotional when I do it. (Ask those who've been in the audience.) I have to say that it was surprisingly easy to admit to such egregious missteps in public, because I know how to take better steps from now on. I followed up Greatest Misses with the Last Word in Better Software's August 2007 where I challenged all agile consultants to emphasize teamwork and all victims of agile consultants to demand help with teamwork. I've made the changes in my practice so far... how many of your readers have done the same?

As for 2014, I've just created a Google Calendar item to write another article on My Greatest Misses: 2008-2014. If Gogle Calendar is still around in seven years, and if I'm still around in seven years, I promise I'll write about it.
